Other causes may include endocrine disorders, chronic infections, and medications. A detailed history, physical examination, and targeted investigations are crucial in narrowing down the differential diagnosis.
Initial investigations should include a complete blood count, metabolic panel, thyroid function tests, and urinalysis. If these are non-revealing, the next step should be guided by the patient's history and physical examination. For example, in the presence of gastrointestinal symptoms, endoscopy or imaging studies may be warranted. In cases where malignancy is suspected, appropriate cancer screening tests should be initiated.
Imaging plays a pivotal role in the evaluation of UWL, especially when malignancy is suspected. Computed tomography (CT) scan of the chest, abdomen, and pelvis can help identify occult malignancies. In selected cases, positron emission tomography (PET) may be beneficial in identifying the primary site of malignancy.
Psychiatric disorders, particularly depression and eating disorders, can be a cause of UWL. A thorough psychiatric evaluation is essential in patients where no physical cause is identified. Referral to a mental health professional may be required in these cases.
In conclusion, a comprehensive approach to diagnosing unexplained weight loss requires a thorough history, physical examination, and targeted investigations. Collaboration with specialists, such as radiologists and psychiatrists, may be necessary to reach a definitive diagnosis. By employing this systematic approach, physicians can accurately diagnose and manage the underlying cause of UWL in their patients.
