As medical professionals, it is crucial to have a comprehensive understanding of neurodegenerative disorders, such as Parkinson's disease (PD). This chronic, progressive condition, second only to Alzheimer's in prevalence, affects approximately 1% of the global population over 60. PD is characterized by dopaminergic neuron loss in the substantia nigra, leading to motor and non-motor symptoms.
PD's primary pathophysiological feature is the progressive loss of dopaminergic neurons in the substantia nigra pars compacta, reducing striatal dopamine levels. This imbalance in the basal ganglia circuitry leads to PD's characteristic motor symptoms. The disease also involves the accumulation of α-synuclein protein in Lewy bodies, although the exact role of these inclusions remains unclear.
The cardinal motor symptoms of PD are bradykinesia, resting tremor, rigidity, and postural instability. Non-motor symptoms such as autonomic dysfunction, cognitive impairment, sleep disorders, and mood disorders can precede motor symptoms, complicating early diagnosis. As the disease advances, patients may also develop complications from treatment, such as motor fluctuations and dyskinesias.
PD diagnosis is primarily clinical, based on history and physical examination. There is currently no definitive diagnostic test, making early diagnosis challenging. Management focuses on symptom control and improving quality of life. Pharmacological treatment primarily involves dopamine replacement or modulation, with Levodopa being the most effective drug. Non-pharmacological interventions include physiotherapy and occupational therapy, and in advanced cases, deep brain stimulation may be considered.
PD is a complex and multifaceted disease with a significant impact on patients' lives. A comprehensive understanding of its pathophysiology, clinical manifestations, diagnosis, and management is crucial in providing optimal patient care. As research advances, it is hoped that new insights into the disease will lead to more effective treatments and ultimately, a cure.
