Initial assessment should involve a thorough history taking, including travel, vaccination status, and exposure to infectious diseases. Physical examination should focus on the characteristics of the rash, associated systemic signs, and potential organ involvement. This information can guide initial diagnostic testing and management.
Differential diagnosis is broad and includes infectious diseases, autoimmune disorders, malignancies, and drug reactions. Infectious causes are the most common and include viral exanthems, bacterial infections, and vector-borne diseases. Autoimmune conditions such as vasculitis and lupus can also present with fever and rash. Malignancies, particularly lymphomas, may manifest similarly. Drug reactions, especially drug hypersensitivity syndromes, should always be considered.
Diagnostic testing should be guided by the clinical suspicion. Basic tests include complete blood count, liver function tests, and inflammatory markers. Specific serological tests, skin biopsy, or imaging may be required based on the suspected diagnosis. In some cases, consultation with dermatology or infectious diseases specialists may be beneficial.
Management is primarily guided by the underlying diagnosis. Symptomatic treatment with antipyretics and antihistamines may be required. In infectious cases, specific antimicrobial therapy is warranted. Autoimmune conditions may necessitate immunosuppressive therapy. Drug-induced causes require discontinuation of the offending agent.
While fever with rash presents a diagnostic dilemma, a systematic approach to history, physical examination, and diagnostic testing can aid in identifying the underlying cause. A broad differential and high clinical suspicion are key to ensuring appropriate management and improved patient outcomes.
