Hypertensive emergencies involve acute elevations in blood pressure, often exceeding 180/120 mmHg, accompanied by evidence of target organ damage. They differ from hypertensive urgencies, which lack such evidence. The key is to identify the organ damage, which can manifest in the cardiovascular, renal, or central nervous systems.
Initial management involves the reduction of blood pressure by 25% within the first hour, followed by a gradual reduction over the next 24 to 48 hours. Rapid reduction beyond this can lead to organ ischemia. Intravenous antihypertensive medications, such as labetalol, nicardipine, or sodium nitroprusside, are typically employed.
Healthcare professionals play a pivotal role in managing hypertensive emergencies. Early recognition, accurate diagnosis, and prompt treatment are crucial. Furthermore, patient education about medication adherence and lifestyle modifications can prevent the recurrence of these emergencies.
