Introduction

Non-Alcoholic Fatty Liver Disease (NAFLD) is an escalating public health issue, affecting approximately one quarter of the global population. Characterised by excessive fat accumulation in the liver, it ranges from simple steatosis to non-alcoholic steatohepatitis (NASH), which can progress to cirrhosis and hepatocellular carcinoma. This article aims to provide an overview of NAFLD, its causes, diagnosis, and modern treatment approaches.

Causes

NAFLD is closely linked with metabolic syndrome, obesity, type 2 diabetes mellitus (T2DM), and dyslipidemia. Genetic predisposition, dietary habits, and sedentary lifestyle are also contributing factors. Notably, NAFLD can occur in individuals with normal body weight, highlighting the influence of metabolic health beyond obesity.

Diagnosis

NAFLD diagnosis is often incidental, discovered through abnormal liver function tests or imaging studies. Ultrasonography is the first-line imaging modality, but lacks sensitivity for mild steatosis and cannot assess fibrosis. Liver biopsy remains the gold standard for diagnosis, but its invasive nature limits its use. Non-invasive tests like transient elastography and serum biomarkers are gaining acceptance for initial assessment and monitoring.

Modern Treatment Approaches

Currently, there's no approved pharmacotherapy for NAFLD. Management focuses on lifestyle modifications to reduce weight and improve metabolic health. In patients with NASH and fibrosis, Vitamin E and Pioglitazone have shown some benefits. Several drugs targeting various pathophysiological aspects of NAFLD are under investigation. Bariatric surgery may be considered in morbidly obese patients with NAFLD.

Conclusion

NAFLD is a complex, multifactorial disease with significant health implications. Understanding its causes and diagnostic challenges is crucial for early detection and management. While lifestyle modifications remain the cornerstone of treatment, ongoing research holds promise for more targeted and effective therapies in the future.
