Chronic cough, defined as a cough persisting for more than eight weeks, is a common yet perplexing clinical challenge. It can significantly impact a patient's quality of life and is often resistant to treatment. This article aims to provide a comprehensive approach to diagnosing and managing chronic cough in clinical practice.
Chronic cough can be attributed to a myriad of causes, often categorized into pulmonary, upper airway, and gastroesophageal disorders. The pathophysiology is multifaceted, involving neuronal hypersensitivity, inflammatory processes, and reflex hypersensitivity. Understanding the underlying mechanisms is crucial for targeted management.
A structured approach to diagnosis is paramount. The initial evaluation should involve a thorough history and physical examination, followed by basic investigations like chest X-ray and spirometry. If the cause remains elusive, more advanced diagnostic tools such as bronchoscopy, high-resolution CT, and esophageal pH monitoring may be warranted. It is crucial to consider less common causes, such as ACE inhibitor-induced cough and psychogenic cough, in refractory cases.
Management should be tailored to the underlying cause. For example, inhaled corticosteroids are beneficial in eosinophilic bronchitis, while proton pump inhibitors are the mainstay of treatment for gastroesophageal reflux disease. Non-pharmacological interventions, including speech therapy and behavioral therapy, can also be effective. Refractory cases may require a multi-disciplinary approach involving pulmonologists, otolaryngologists, and gastroenterologists.
Chronic cough is a complex clinical entity, requiring a comprehensive and systematic approach to diagnosis and management. By understanding the etiology and pathophysiology, employing a structured diagnostic approach, and tailoring management to the underlying cause, clinicians can improve patient outcomes and quality of life. Future research should focus on novel therapeutic targets and the development of predictive models for chronic cough.
