These digital tools, rooted in data science and informatics, enable ongoing monitoring, personalized interventions, and enhanced care coordination, addressing the chronic and relapsing nature of addiction. This review examines the epidemiology of SUDs, pathophysiological mechanisms underlying addiction, risk factors, clinical features, diagnostic approaches, and evidence-based treatments, with a focus on the clinical implementation of informatics platforms. Recent advances, emerging digital therapies, and guideline recommendations are discussed, highlighting the potential and limitations of recovery informatics in supporting sustained remission and improved patient outcomes.
Substance use disorders represent a significant and growing public health challenge worldwide, characterized by chronicity, high relapse rates, and complex biopsychosocial dynamics. Traditional treatment models often fall short in addressing the longitudinal needs of individuals, necessitating innovative approaches that leverage technology. Recovery informatics platforms are emerging as vital tools in bridging gaps in care, enabling real-time data capture, proactive management, and tailored patient engagement strategies. This article reviews the scientific foundation and clinical application of these platforms in the continuum of addiction care.
The global burden of SUDs is substantial, with the World Health Organization estimating over 35 million people requiring treatment for drug use disorders. In the United States, opioid use disorder alone accounted for over 70,000 overdose deaths in 2022. The chronic relapsing nature of SUDs contributes to high healthcare utilization, psychosocial morbidity, and mortality. Epidemiological studies underscore the need for long-term management strategies that extend beyond acute detoxification, emphasizing the importance of sustained recovery support and monitoring—domains where informatics platforms can have a profound impact.
Addiction arises from complex neurobiological processes involving dopaminergic reward pathways, neuroadaptations in the prefrontal cortex, and dysregulation of stress and motivation circuits. Genetic predisposition, environmental exposures, and psychosocial stressors interact to drive the transition from voluntary use to compulsive drug-seeking. Chronic substance exposure induces changes in synaptic plasticity, stress response, and executive function, underpinning the persistent vulnerability to relapse. Understanding these mechanisms guides the development of informatics tools capable of tracking behavioral and biological risk markers in real time.
Risk factors for addiction include genetic susceptibility, early exposure to substances, psychiatric comorbidities (e.g., depression, anxiety, trauma), adverse childhood experiences, and social determinants of health such as poverty and lack of social support. Informatics platforms can aggregate and analyze these multidimensional risk factors, facilitating risk stratification and personalized care planning. Digital phenotyping—utilizing data from mobile devices and electronic health records—offers novel approaches for identifying individuals at increased risk of relapse or adverse outcomes.
Clinically, SUDs are characterized by impaired control over substance use, craving, withdrawal symptoms, tolerance, and functional impairment across domains. Co-occurring psychiatric and medical conditions are common, complicating assessment and management. Informatics platforms can support symptom monitoring through ecological momentary assessments, wearable sensors, and patient-reported outcomes, enabling timely intervention and enhancing self-management capabilities.
Diagnosis of SUDs is based on standardized criteria such as DSM-5 or ICD-11, incorporating clinical interviews, validated screening tools, and collateral information. Informatics solutions can automate the collection and synthesis of diagnostic data, improve accuracy, and facilitate early detection. Integration with electronic health records allows for longitudinal tracking of symptom trajectories, medication adherence, and response to interventions, supporting measurement-based care.
Evidence-based treatment of SUDs includes pharmacotherapy (e.g., buprenorphine, methadone, naltrexone for opioid use disorder), psychosocial interventions (cognitive-behavioral therapy, motivational interviewing), and recovery support services. Recovery informatics platforms enhance treatment delivery by enabling remote monitoring, digital therapeutic interventions, automated appointment reminders, and secure messaging between patients and providers. Such platforms can facilitate stepped-care approaches, promptly identify lapses, and tailor interventions based on real-time data.
The past decade has seen rapid evolution in digital health, with the emergence of smartphone apps, telemedicine solutions, and artificial intelligence-driven analytics. Recent advances include machine learning algorithms for relapse prediction, wearable biosensors for physiologic monitoring, and digital cognitive-behavioral therapy modules. Integration of peer support networks and gamification elements within informatics platforms has shown promise in enhancing engagement and retention. Ongoing research explores the utility of just-in-time adaptive interventions (JITAI) and natural language processing for dynamic risk assessment.
Major clinical guidelines, including those from the American Society of Addiction Medicine (ASAM) and the Substance Abuse and Mental Health Services Administration (SAMHSA), increasingly recognize the role of technology in SUD care. Recommendations emphasize the integration of informatics platforms for care coordination, outcome monitoring, and patient engagement, provided they comply with privacy, confidentiality, and ethical standards. Guidelines highlight the importance of interoperability with electronic health records and the need for ongoing evaluation of digital tool efficacy and patient safety.
Recovery informatics platforms represent a transformative advancement in long-term addiction care, offering scalable, data-driven approaches to support sustained recovery. By enabling continuous monitoring, personalized interventions, and enhanced coordination, these tools address critical gaps in traditional care models. Ongoing research, robust implementation, and adherence to clinical guidelines will be essential to realize the full potential of informatics in improving outcomes for individuals with substance use disorders.
