Unexplained weight loss (UWL) is a diagnostic challenge in clinical practice. It is a potential indicator of serious underlying disease, often signaling malignancy, gastrointestinal disorders, or endocrine abnormalities. A systematic and comprehensive approach is essential to identify the cause and initiate appropriate management.
The initial evaluation of UWL should involve a detailed history and thorough physical examination. Clinicians should inquire about changes in appetite, dietary intake, and physical activity. History of associated symptoms, medication use, and psychosocial factors should also be considered. Physical examination can provide clues to systemic diseases such as malignancies or chronic infections.
Initial laboratory investigations should include complete blood count, comprehensive metabolic panel, thyroid function tests, and inflammatory markers. Imaging studies, such as chest radiographs and abdominal ultrasound, may be helpful based on the patient's symptoms and risk factors. Further diagnostic tests should be guided by initial findings and clinical suspicion.
In elderly patients, UWL may be the only manifestation of a serious disease. Therefore, a high index of suspicion is required. In patients with psychiatric history, disorders such as depression and eating disorders should be considered. In cases where no clear cause is identified despite extensive evaluation, a diagnosis of idiopathic UWL may be considered, and close follow-up is recommended.
Unexplained weight loss is a common but challenging clinical scenario. It requires a systematic and comprehensive approach to evaluation, including detailed history, physical examination, and judicious use of laboratory and imaging studies. Special considerations are needed for elderly patients and those with psychiatric history. Clinicians should maintain a high index of suspicion for serious underlying diseases and ensure close follow-up for patients with idiopathic UWL.
