Peptic Ulcer Disease (PUD) continues to pose significant clinical challenges due to its recurrent nature and potential complications. However, recent advancements in diagnosis and treatment strategies have revolutionized PUD management, improving patient outcomes significantly.
Endoscopy remains the gold standard for PUD diagnosis, offering direct visualization and biopsy capabilities. Recent advancements include the use of Narrow Band Imaging (NBI) and Magnifying Endoscopy (ME) to enhance lesion detection and characterization. Furthermore, non-invasive tests like Urea Breath Test (UBT) and Stool Antigen Test (SAT) have improved Helicobacter pylori detection, a primary PUD cause.
Proton Pump Inhibitors (PPIs) remain the cornerstone of PUD treatment. However, novel therapies such as Vonoprazan, a Potassium-Competitive Acid Blocker (P-CAB), have shown promising results with faster ulcer healing rates. In H. pylori-associated ulcers, quadruple therapy including bismuth and newer antibiotics like rifabutin have improved eradication rates.
Surgical intervention in PUD has significantly decreased due to effective medical therapies. However, it remains crucial for managing complications like bleeding, perforation, and gastric outlet obstruction. Laparoscopic and endoscopic techniques have minimized surgical morbidity and improved recovery times.
Best practices in PUD management involve a comprehensive approach including effective patient education, lifestyle modifications, and regular follow-ups. The use of risk scoring systems like the Glasgow-Blatchford score can help stratify patients and guide management. Moreover, H. pylori eradication should be confirmed post-treatment to prevent recurrence.
Advancements in PUD management have transformed patient care, emphasizing early diagnosis, effective treatment, and prevention of complications. As healthcare professionals, it is imperative to stay abreast with these advancements and incorporate best practices in our approach to PUD management.
