Pulmonary Function Testing (PFT) is a fundamental tool in diagnosing and managing patients with respiratory disorders. Despite its importance, many healthcare professionals find PFT interpretation challenging. This article aims to provide a comprehensive guide to understanding and interpreting PFT results.
The key components of PFT include spirometry, lung volume measurements, and diffusion capacity. Spirometry assesses the airflow and volume of the lungs, providing values such as Forced Vital Capacity (FVC) and Forced Expiratory Volume in one second (FEV1). Lung volume measurements, including Total Lung Capacity (TLC), provide insights into restrictive lung diseases. The diffusion capacity measures how well oxygen moves from the lungs to the bloodstream.
Spirometry readings primarily indicate obstructive or restrictive lung diseases. An FEV1/FVC ratio less than 0.7 suggests obstructive disease. A reduced FVC with a normal or increased FEV1/FVC ratio suggests restrictive disease. However, these are general guidelines and clinical correlation is essential.
Reduced TLC indicates restrictive lung disease. An increased Residual Volume (RV) may suggest air trapping, common in obstructive diseases. A low Diffusing Capacity of the Lung for Carbon Monoxide (DLCO) may suggest interstitial lung disease or pulmonary vascular disease, but can also be reduced in emphysema.
While PFT provides valuable information, its interpretation should always be correlated with the patient's clinical picture. Symptoms, physical examination findings, and other investigations should be considered to make an accurate diagnosis.
Interpreting PFTs can be complex, but understanding the individual components and their implications can improve diagnostic accuracy and patient care. It is essential to integrate PFT results with the overall clinical context to make meaningful interpretations.
