Acute Coronary Syndrome (ACS) remains a leading cause of morbidity and mortality worldwide. With the advent of new technologies and therapeutic approaches, the landscape of ACS management is continually evolving. This article aims to discuss the latest advancements and best practices in ACS management.
High-sensitivity cardiac troponin assays have revolutionized the early detection of ACS. These assays allow for quicker rule-in and rule-out of myocardial infarction, leading to more efficient management and better patient outcomes. The use of coronary computed tomography angiography (CCTA) is also gaining traction, providing non-invasive visualization of coronary anatomy and plaque characteristics.
Newer P2Y12 inhibitors, such as ticagrelor and prasugrel, have shown superiority over clopidogrel in reducing cardiovascular events. The advent of potent anticoagulants, like rivaroxaban, has also reshaped the ACS therapeutic landscape. Additionally, the use of PCSK9 inhibitors for LDL-C reduction is promising, especially in high-risk patients.
While primary percutaneous coronary intervention (PCI) remains the gold standard for ST-segment elevation myocardial infarction (STEMI), the choice between PCI and coronary artery bypass grafting (CABG) in non-STEMI is less clear. Recent studies suggest that an individualized approach, considering patient characteristics and coronary anatomy, may yield better outcomes.
Secondary prevention measures, including lifestyle modifications and medication adherence, are crucial to prevent recurrent events. Multidisciplinary cardiac rehabilitation programs have been shown to improve long-term prognosis and should be considered in all ACS patients.
Advancements in diagnostic tools, therapeutic options, and revascularization strategies have improved ACS management significantly. However, a comprehensive approach, including robust secondary prevention measures, is essential to optimize patient outcomes. As medical professionals, it is imperative to stay updated with these advancements and implement best practices in our clinical practice.
