Elevated liver enzymes are a common clinical finding, often discovered incidentally during routine screening. They may indicate underlying liver disease, but can also result from non-hepatic conditions. Understanding the implications of elevated liver enzymes and their appropriate management is crucial for clinicians to deliver optimal patient care.
Liver enzymes, primarily alanine aminotransferase (ALT) and aspartate aminotransferase (AST), are sensitive markers of hepatocellular injury. However, their specificity is limited, as they can also be elevated in conditions affecting other organs. Additionally, the degree of elevation does not necessarily correlate with the severity of liver disease. Therefore, interpretation of liver enzyme levels should be performed in the context of the overall clinical picture.
Common causes of elevated liver enzymes include viral hepatitis, alcohol-related liver disease, non-alcoholic fatty liver disease (NAFLD), and drug-induced liver injury. Less common causes include autoimmune hepatitis, hemochromatosis, Wilson's disease, and alpha-1 antitrypsin deficiency. Non-hepatic causes such as myocardial infarction, muscle damage, and thyroid disorders can also result in elevated liver enzymes.
A comprehensive history and physical examination are the first steps in evaluating elevated liver enzymes. The history should focus on risk factors for liver disease, such as alcohol use, medication history, travel history, and family history of liver disease. The physical examination should look for signs of chronic liver disease, such as jaundice, ascites, and hepatomegaly.
Initial laboratory evaluation should include a complete blood count, coagulation studies, and liver function tests. Depending on the clinical context, additional tests may be needed, such as viral hepatitis serologies, iron studies, autoimmune markers, and genetic testing. Imaging studies, such as ultrasound, computed tomography (CT), or magnetic resonance imaging (MRI), may also be useful.
Management should be directed towards the underlying cause of the elevated liver enzymes. This may involve lifestyle modifications, medication changes, or specific treatments for liver disease. In some cases, referral to a hepatologist may be necessary.
Follow-up testing is crucial to monitor the response to treatment and to detect any progression of liver disease. The frequency of follow-up and the need for liver biopsy depend on the etiology and severity of liver disease.
