Recent research underscores the importance of multidomain interventions—encompassing nutrition, physical activity, cognitive stimulation, sleep hygiene, and social engagement—in optimizing healthspan and delaying the onset of age-related morbidity. This review synthesizes current evidence on the mechanisms, clinical relevance, and practical applications of multidomain lifestyle optimization for healthy aging, with a focus on risk factor modification, early identification, and evidence-based management strategies.
As global populations age, the pursuit of strategies to promote healthy aging becomes a public health imperative. Healthy aging is not solely the absence of disease but the maintenance of functional ability and quality of life. Multidomain lifestyle optimization—targeting multiple interrelated behavioral factors—holds promise for reducing the burden of chronic disease and disability. This article examines the scientific rationale, epidemiological trends, and clinical implications of a comprehensive lifestyle approach to healthy aging.
The World Health Organization projects that by 2050, the number of people aged 60 years and older will double, reaching 2.1 billion globally. Age-associated conditions such as cardiovascular disease, diabetes, neurodegenerative disorders, osteoporosis, and sarcopenia account for the majority of morbidity and mortality in older adults. Epidemiological studies demonstrate that unhealthy lifestyle behaviors—physical inactivity, poor diet, smoking, excessive alcohol use, and social isolation—significantly increase the risk of premature aging and multimorbidity. The clinical manifestations of unhealthy aging encompass functional decline (e.g., impaired mobility, frailty), cognitive impairment, mood disturbances, sarcopenia, osteoporosis, and increased susceptibility to infection and chronic diseases. Early identification of subtle changes in physical, cognitive, or emotional health is critical for timely intervention and prevention of further decline.
Diagnosis of age-related decline involves comprehensive geriatric assessment, including evaluation of physical function (e.g., gait speed, handgrip strength), cognitive screening (e.g., MMSE, MoCA), nutritional status, sleep quality, and psychosocial well-being. Biomarkers of aging, such as inflammatory markers (CRP, IL-6), telomere length, and metabolic parameters, provide additional insight into biological aging and intervention efficacy. Risk stratification tools and digital health technologies are increasingly utilized for early detection and personalized management.
Multidomain lifestyle optimization integrates evidence-based interventions across diet, physical activity, cognitive engagement, sleep, and social interaction. Nutritional recommendations emphasize Mediterranean or DASH-style diets rich in fruits, vegetables, whole grains, lean proteins, and healthy fats. Regular aerobic and resistance exercise is critical for maintaining muscle mass, cardiovascular health, and neuroplasticity. Cognitive training, lifelong learning, mindfulness, and social activities bolster mental resilience and delay cognitive decline. Sleep hygiene interventions target circadian alignment and reduce sleep fragmentation. Psychosocial support and community engagement address loneliness and depression, enhancing overall well-being.
Recent clinical trials, such as the FINGER and MAPT studies, provide robust evidence for the efficacy of multidomain interventions in delaying cognitive decline and preserving function in older adults. Digital health platforms, wearable technologies, and remote monitoring enable personalized feedback and adherence tracking. Advances in precision medicine—including pharmacogenomics and biomarker-guided lifestyle interventions—are paving the way for individualized aging trajectories. Novel therapies targeting senescent cells (senolytics), gut microbiome modulation, and gene editing hold potential for future integration with lifestyle optimization strategies.
Leading organizations, including the WHO, American Heart Association, and European Society of Cardiology, endorse a multidomain approach to healthy aging. Guidelines recommend regular physical activity (150 minutes/week of moderate-intensity exercise), adherence to nutrient-dense dietary patterns, cognitive and social engagement, and routine screening for age-related risk factors. Multidisciplinary care teams and patient-centered care plans are emphasized to support sustained lifestyle change and address barriers to implementation.
Multidomain lifestyle optimization represents a cornerstone of healthy aging, with compelling evidence supporting its role in reducing disease burden, preserving functional independence, and enhancing quality of life. A proactive, integrative approach—tailored to individual risk profiles and supported by interdisciplinary collaboration—should be prioritized in clinical practice to promote longevity and vitality in aging populations. Ongoing research and innovation will further refine and personalize strategies for optimal aging outcomes.
