The rising global prevalence of AD, particularly in industrialized nations, underscores the need for updated evidence-based strategies in diagnosis, management, and long-term care. This review synthesizes current epidemiological trends, elucidates underlying pathophysiological mechanisms, and details risk factors and clinical manifestations of AD. We examine contemporary and emerging treatment modalities, with an emphasis on targeted biologics and novel small-molecule therapies, and provide practical insights into guideline-aligned, individualized care. The article concludes with expert commentary on evolving clinical recommendations and future directions in the management of atopic dermatitis.

Introduction

Atopic dermatitis is a multifaceted skin disease characterized by recurrent eczematous lesions, intense pruritus, and a relapsing course. Its chronic nature and early onset contribute to significant psychosocial and economic burdens for patients and healthcare systems. Recent decades have witnessed a steady increase in AD prevalence, particularly among children, necessitating a re-examination of therapeutic approaches and long-term management strategies. Clinicians are challenged by the heterogeneity of disease presentation, comorbidities, and the need for individualized, evidence-based care. This review aims to provide an in-depth, clinically relevant summary of the latest advances and practical considerations in the management of atopic dermatitis for healthcare professionals.

Epidemiology / Disease Burden

Globally, the prevalence of atopic dermatitis has increased significantly, with the highest rates observed in high-income, urbanized countries. Approximately 15-20% of children and 2-10% of adults are affected, with many cases persisting into adulthood or relapsing after periods of remission. The disease imposes a substantial burden through direct medical costs, loss of productivity, and reduced quality of life. AD is associated with sleep disruption, psychological distress, and increased risk of comorbid allergic conditions such as asthma and allergic rhinitis. Distribution patterns differ: infants typically present with facial and extensor involvement, whereas older children and adults exhibit flexural predilection. Secondary changes such as excoriations, crusting, and post-inflammatory hyperpigmentation are common. Recurrent infections and ocular, respiratory, or gastrointestinal comorbidities may complicate the disease course. Assessment tools such as SCORAD and EASI provide standardized measures for disease severity and therapeutic response.

Diagnosis

Diagnosis of atopic dermatitis is primarily clinical, based on established criteria such as those of Hanifin and Rajka, and the UK Working Party. Key diagnostic features include chronic or relapsing dermatitis, typical morphology and distribution, and a personal or family history of atopy. Laboratory testing, including total and specific IgE levels, may support diagnosis in selected cases but is not routinely required. Differential diagnoses to consider include seborrheic dermatitis, contact dermatitis, psoriasis, and cutaneous T-cell lymphoma. Skin biopsy is reserved for atypical or refractory presentations.

Treatment & Management

Management of AD requires a comprehensive, patient-centered approach encompassing education, skin barrier restoration, anti-inflammatory therapy, and trigger avoidance. Regular use of emollients forms the cornerstone of skin care, while topical corticosteroids and calcineurin inhibitors address acute inflammation. Antihistamines may provide symptomatic relief for pruritus but have limited efficacy for skin lesions. In moderate-to-severe cases, phototherapy or systemic immunomodulators, such as cyclosporine or methotrexate, may be indicated. Multidisciplinary care, including psychological support and allergist consultation, is essential for patients with severe or refractory disease.

Recent Advances / Emerging Therapies

The therapeutic landscape of AD has expanded with the advent of targeted biologics and small-molecule inhibitors. Dupilumab, a monoclonal antibody against the IL-4 receptor alpha subunit, has demonstrated efficacy in reducing inflammation, pruritus, and improving quality of life in moderate-to-severe AD. Additional biologics targeting IL-13 (tralokinumab, lebrikizumab) and JAK inhibitors (upadacitinib, abrocitinib) offer further options, particularly for patients with inadequate response or contraindications to conventional therapies. These agents provide durable control with favorable safety profiles, although long-term data and cost-effectiveness analyses are ongoing. Real-world integration of these therapies requires individualized risk-benefit assessment and careful monitoring for adverse effects.

Guideline Recommendations

Recent international and national guidelines emphasize the importance of personalized, stepwise therapy for AD. First-line management includes skin hydration, avoidance of triggers, and topical anti-inflammatories. For refractory or severe cases, escalation to phototherapy, systemic immunosuppressants, or biologic agents is recommended. Ongoing assessment of disease severity, treatment response, and quality of life should inform therapeutic adjustments. Guidelines also highlight the need for patient education, adherence support, and multidisciplinary collaboration to optimize long-term outcomes. Vaccination status, infection risk, and comorbidity management are important considerations in systemic and biologic therapy candidates.

The rising burden of atopic dermatitis necessitates an integrated approach encompassing early recognition, mechanism-based therapy, and long-term care strategies. Advances in understanding the immunopathogenesis of AD have catalyzed the development of targeted biologics and small-molecule inhibitors, transforming the therapeutic paradigm. Ongoing research into disease-modifying interventions, biomarkers, and prevention strategies holds promise for further improving patient outcomes. Clinicians must remain abreast of evolving evidence and guideline recommendations to deliver comprehensive, individualized care for patients with atopic dermatitis.
