The aim is to provide clinicians with comprehensive, practical insights for optimizing care in this pivotal life stage.
Women's health during adolescence is profoundly influenced by intricate interactions between developmental and hormonal processes. The transition from childhood to adulthood involves activation of the hypothalamic-pituitary-gonadal (HPG) axis, leading to pubertal onset, sexual maturation, and reproductive capability. These changes are accompanied by psychosocial adaptation and evolving healthcare needs. Understanding these physiological milestones and their clinical implications is essential for effective adolescent care. This article reviews the epidemiological patterns, hormonal mechanisms, and evidence-based approaches that define adolescent women's health, with a focus on conditions such as menstrual disorders, polycystic ovary syndrome (PCOS), and the impact of hormonal contraception.
Adolescent girls represent a significant global demographic, with approximately 1.2 billion individuals aged 10-19 years. Menstrual disorders, such as dysmenorrhea (affecting up to 90% of adolescents) and abnormal uterine bleeding (AUB), are among the most frequent gynecological complaints. PCOS prevalence in this age group is estimated at 6-10%, though diagnostic criteria remain a topic of debate. Furthermore, adolescent reproductive health challenges contribute to a substantial burden of disease, including iron-deficiency anemia, unintended pregnancies, and sexually transmitted infections (STIs). Recognizing the high prevalence and impact of these disorders is crucial for early intervention and prevention of long-term sequelae.

Pubertal disorders may manifest as delayed or precocious onset of secondary sexual characteristics. Diagnostic evaluation in adolescent females prioritizes exclusion of secondary causes of menstrual irregularities, such as thyroid dysfunction, hyperprolactinemia, and structural anomalies. Laboratory investigations may include serum FSH, LH, estradiol, androgens, TSH, and prolactin, alongside metabolic screening for insulin resistance and dyslipidemia. Pelvic ultrasonography aids in assessing ovarian morphology and uterine development. Importantly, diagnostic criteria for PCOS in adolescents require persistent symptoms for at least two years post-menarche and careful exclusion of physiological pubertal variations. A multidisciplinary approach, incorporating endocrinology, gynecology, and psychology, is often warranted.
Management strategies are individualized, addressing both symptom control and underlying pathophysiology.
