Vertigo, a sensation of feeling off balance, is often associated with a condition known as vestibular disorder. This condition, marked by dizziness, is a common complaint in clinical practice and can pose diagnostic challenges to healthcare professionals due to its multifactorial etiology.
Vertigo is a subtype of dizziness characterized by a sensation of spinning or a feeling that one's surroundings are spinning. It is often caused by issues in the brain or the inner ear, including benign paroxysmal positional vertigo (BPPV), Meniere's disease, or vestibular neuritis. Understanding the underlying pathophysiology is essential for effective diagnosis and management.
A comprehensive clinical approach to vertigo begins with a detailed history and physical examination. The history should focus on the onset, duration, frequency, and nature of the vertigo attacks. Physical examination including neurological and otological examination can help identify central or peripheral causes. Further, bedside tests like the Dix-Hallpike maneuver or head impulse test can be instrumental in diagnosis.
While history and physical examination guide the initial evaluation, diagnostic imaging and laboratory tests are useful adjuncts. MRI and CT scans can help identify central causes like stroke or tumor. Audiometry can detect associated hearing loss, common in Meniere's disease.
Treatment of vertigo is largely dependent on the underlying cause. Vestibular rehabilitation exercises are beneficial for most types of vertigo. Pharmacotherapy with drugs like betahistine or prochlorperazine can control symptoms. In refractory cases, surgical interventions may be considered.
Vertigo presents a complex clinical challenge requiring a comprehensive and systematic approach for effective management. A detailed understanding of its pathophysiology, coupled with a thorough clinical evaluation and appropriate use of diagnostic aids, can help healthcare professionals provide effective and timely treatment to patients suffering from this debilitating condition.
