Anemia, a condition characterized by a decrease in the number of red blood cells or less than the normal quantity of hemoglobin in the blood, can be a diagnostic challenge due to its multifactorial etiologies. This guide aims to provide a comprehensive approach to the diagnosis of anemia.
The first step in the diagnostic process is a thorough patient history and physical examination. Key factors to consider include symptoms, duration, triggers, and family history. It's crucial to assess dietary habits, medications, and any history of bleeding. Physical examination may reveal pallor, jaundice, or signs of specific causes like splenomegaly.
Initial laboratory tests should include a complete blood count (CBC) and reticulocyte count. A low hemoglobin level confirms anemia, while the reticiculocyte count helps determine if the anemia is regenerative or non-regenerative. Further tests may include iron studies, vitamin B12, folate levels, and a peripheral blood smear.
Anemia can be classified based on the mean corpuscular volume (MCV) into microcytic, normocytic, and macrocytic types. Microcytic anemia is usually due to iron deficiency or thalassemia. Normocytic anemia can be caused by chronic disease, acute blood loss, or bone marrow failure. Macrocytic anemia is often due to vitamin B12 or folate deficiency.
When initial investigations do not yield a clear diagnosis, more advanced procedures may be required. Bone marrow biopsy can be helpful in evaluating unexplained normocytic anemia or suspected bone marrow disorders. Endoscopy or colonoscopy may be needed to identify sources of occult bleeding.
Anemia presents a diagnostic challenge due to its diverse causes. A systematic approach involving a thorough history, physical examination, appropriate laboratory tests, and classification based on MCV can streamline the diagnostic process. In complex cases, advanced procedures may be necessary to identify the underlying cause.
