Pregnancy presents unique physiological and pathophysiological challenges that complicate emergency care. Acute pregnancy-related medical conditions require a nuanced, evidence-based approach to optimize maternal and fetal outcomes. This review explores the epidemiology, pathophysiology, risk stratification, clinical features, diagnostic dilemmas, and management strategies for common and critical pregnancy-related emergencies, drawing on recent scientific literature and current clinical guidelines. Special emphasis is placed on conditions such as preeclampsia, eclampsia, thromboembolic disease, acute abdomen, and severe infections. The article aims to provide clinicians with updated, clinically relevant insights to improve timely recognition and effective intervention in high-risk obstetric populations.
Emergency medical care for pregnant patients is complex due to the dual consideration of maternal and fetal health and the unique physiological changes that occur during gestation. Standard diagnostic and therapeutic pathways may not always be directly applicable, and delays or missteps can have serious consequences. The spectrum of pregnancy-related emergencies encompasses hypertensive disorders, thromboembolic events, sepsis, acute abdominal conditions, and exacerbations of chronic illness. This review synthesizes current literature and clinical guidelines to elucidate the distinctive challenges encountered in the emergency care of pregnant women, emphasizing the need for prompt recognition, multidisciplinary collaboration, and evidence-based interventions.
Globally, pregnancy-related medical emergencies contribute significantly to maternal morbidity and mortality. The World Health Organization estimates over 295,000 maternal deaths annually, with hypertensive disorders, hemorrhage, and infections as leading causes. In high-resource settings, maternal mortality rates have plateaued or risen, driven by delayed recognition and management of acute conditions. Thromboembolic disease is four to five times more likely in pregnancy, while hypertensive disorders complicate approximately 5–10% of pregnancies. The burden is disproportionately higher among women with socioeconomic disadvantages, limited prenatal care, or comorbid conditions. Emergency departments must remain vigilant, as nearly 10% of pregnant women will present with acute complaints requiring urgent evaluation during gestation.
Pregnancy induces profound cardiovascular, hematologic, respiratory, and renal adaptations. Plasma volume expands by up to 50%, systemic vascular resistance decreases, and cardiac output rises. These changes can mask or mimic symptoms of acute pathology. The hypercoagulable state, characterized by elevated fibrinogen and clotting factors, underpins the increased risk for venous thromboembolism (VTE). Immunological modulation in pregnancy increases susceptibility to certain infections and alters inflammatory responses. The placenta plays a critical role in pathologies such as preeclampsia and acute fatty liver of pregnancy by releasing vasoactive substances and pro-inflammatory mediators. These mechanisms require clinicians to recalibrate their diagnostic acumen and therapeutic thresholds in the emergency setting.
Risk stratification is essential for anticipating and managing pregnancy emergencies. Advanced maternal age, obesity, preexisting hypertension, diabetes, history of thromboembolism, multifetal gestation, and limited prenatal care markedly increase the risk of complications. Socioeconomic disparities, substance use, and prior obstetric complications further exacerbate vulnerability. Genetic factors, such as thrombophilias, also predispose to VTE and hypertensive disorders. Identification of at-risk individuals during prenatal care and upon presentation to emergency services is critical for early intervention.
Presentation of acute pregnancy-related conditions is often nonspecific. Hypertensive disorders may manifest as headache, visual changes, or right upper quadrant pain, while eclampsia is heralded by seizures. VTE may present with unilateral leg swelling or sudden dyspnea. Acute abdomen in pregnancy due to appendicitis, cholecystitis, or uterine rupture can be challenging to distinguish from physiological discomforts. Sepsis may be insidious, with tachycardia or mild pyrexia as early signs. Importantly, vital sign thresholds may differ in pregnancy; for example, compensated shock may occur with relatively normal blood pressure. High suspicion and systematic assessment are paramount.
Diagnostic evaluation in pregnant patients must balance accuracy with fetal safety. Laboratory markers including liver function tests, renal panels, and coagulation profiles are essential, but normal pregnancy ranges must be considered. Imaging selection is guided by clinical urgency and fetal exposure risks. Ultrasound is the first-line modality for abdominal and pelvic evaluation, while magnetic resonance imaging is preferred for complex cases. When necessary, computed tomography may be justified, using shielding and the lowest possible radiation dose. Point-of-care testing such as urinalysis for proteinuria or bedside echocardiography supplements rapid diagnosis. Clinicians should maintain a low threshold for diagnostic escalation in deteriorating patients.
Management principles revolve around maternal stabilization, fetal assessment, and targeted therapy. In hypertensive emergencies, first-line agents include labetalol and hydralazine; magnesium sulfate is essential for seizure prophylaxis in preeclampsia and treatment of eclampsia. Anticoagulation for VTE typically involves low molecular weight heparin, given its safety profile. Sepsis management follows standard protocols with early antibiotics and source control, tailored to likely pathogens in pregnancy. Acute abdomen may require surgical intervention, with multidisciplinary input from obstetrics, surgery, and anesthesia. Fetal monitoring and readiness for emergent delivery are integral in all severe cases. Fluid resuscitation and oxygen therapy must be judicious, accounting for altered maternal physiology.
Recent advances have refined emergency pregnancy care. Angiogenic markers like sFlt-1/PlGF ratios aid early diagnosis of preeclampsia. Novel oral anticoagulants are being studied, though not yet standard in pregnancy. Point-of-care ultrasound has improved rapid bedside assessment of maternal and fetal status. Enhanced sepsis protocols, incorporating maternal early warning systems, have reduced mortality. Telemedicine is emerging as a tool for remote triage and specialist consultation. Research into biomarkers and personalized therapy continues to evolve, promising better risk stratification and individualized management in the near future.
International and national guidelines underscore early recognition, risk assessment, and multidisciplinary management. The American College of Obstetricians and Gynecologists recommends prompt antihypertensive therapy in severe hypertension, immediate magnesium sulfate for eclampsia, and low molecular weight heparin for VTE. Sepsis guidelines emphasize timely antibiotics and source control. Diagnostic imaging should proceed when benefits outweigh risks, with shielding and dose minimization. Regular simulation training in obstetric emergencies and implementation of maternal early warning systems are strongly advocated to improve outcomes.
Emergency care of pregnancy-related medical conditions requires a high degree of vigilance, interdisciplinary collaboration, and adherence to evidence-based protocols. Unique physiological adaptations, overlapping symptomatology, and the imperative to safeguard both maternal and fetal health complicate clinical decision-making. Recent advances in diagnostics and therapy, alongside robust guideline frameworks, offer avenues for improved outcomes. Ongoing research, education, and system-level innovations are essential to address persistent challenges and disparities in maternal emergency care.
