Acute Coronary Syndrome (ACS) remains a significant cause of morbidity and mortality worldwide, necessitating constant advancements in management strategies. This guide aims to provide a comprehensive overview of the current best practices and advancements in the treatment of ACS.
Pharmacotherapy remains a cornerstone in ACS management. The use of P2Y12 inhibitors, such as clopidogrel, prasugrel, and ticagrelor, has revolutionized antiplatelet therapy. Newer agents like cangrelor offer rapid and potent platelet inhibition. The introduction of novel oral anticoagulants (NOACs) has also improved patient outcomes, offering a safer alternative to traditional anticoagulants with a lower bleeding risk.
Primary percutaneous coronary intervention (PCI) remains the gold standard for ST-segment elevation myocardial infarction (STEMI). Advances in stent technology, such as bioresorbable vascular scaffolds and drug-eluting stents, have improved long-term outcomes. Furthermore, the advent of mechanical circulatory support devices, such as intra-aortic balloon pumps and percutaneous ventricular assist devices, have been lifesaving in patients with cardiogenic shock.
Early recognition and prompt reperfusion therapy are key in ACS management. Risk stratification using tools like the GRACE score helps guide management. Dual antiplatelet therapy (DAPT), high-intensity statin therapy, and aggressive blood pressure control form the mainstay of secondary prevention. The importance of cardiac rehabilitation and lifestyle modifications should also be emphasized to patients.
Management of ACS has evolved significantly over the years, with numerous advancements improving patient outcomes. It is crucial for healthcare professionals to stay updated with these advancements and best practices to provide optimal care for their patients. A multidimensional approach, including pharmacological interventions, invasive strategies, and lifestyle modifications, remains the cornerstone of ACS management.
