Behavioral science offers a multidimensional approach to promoting blood donation, addressing not only individual motivation but also broader societal, psychological, and organizational factors. This review synthesizes current evidence on the application of behavioral science to optimize blood donor recruitment, retention, and overall system sustainability, providing clinically relevant insights and highlighting recent advances in the field.
The sustainability of blood donation programs is a public health imperative, underpinning the success of surgical, trauma, oncology, and hematology services worldwide. Traditional approaches to donor recruitment have plateaued in effectiveness, necessitating innovative behavioral strategies grounded in scientific evidence. Behavioral science, involving psychology, sociology, and decision science, enables a more nuanced understanding of donor motivation and barriers, informing effective interventions to secure a reliable blood supply.
Transfusion medicine relies on voluntary, non-remunerated blood donors, yet global statistics reveal persistent mismatches between supply and demand. The World Health Organization estimates that over 118.5 million blood donations are collected annually, but shortages remain acute in low- and middle-income countries, and periodic shortages occur even in high-resource settings. Aging populations and the increased prevalence of chronic diseases intensify demand, while donor pools in many countries are shrinking due to demographic trends and changing social behaviors. Seasonal and crisis-driven fluctuations further exacerbate supply instability, underscoring the need for robust, sustainable donor systems.
While blood donation itself is not a disease state, understanding the physiological and psychological pathways influencing donor behavior is crucial. Evidence-based recommendations emphasize the importance of a positive donation experience, transparent communication regarding blood use and safety, and the removal of logistical barriers. Active monitoring of donor satisfaction and adverse events, combined with data-driven behavioral interventions, is considered best practice for sustainable blood supply management.
Behavioral science provides critical insights and evidence-based strategies for enhancing the sustainability of blood donation systems. Addressing the complex interplay of individual, social, and organizational factors can significantly improve donor recruitment and retention, ultimately safeguarding the blood supply for current and future clinical needs. Continued research, innovation, and the integration of behavioral science into policy and practice are imperative to meet the evolving challenges of transfusion medicine.
