Acute chest pain (ACP) is a common presenting symptom in clinical practice, often posing a diagnostic challenge for clinicians. It is crucial to promptly identify its cause, as it may be a manifestation of life-threatening conditions such as acute coronary syndrome (ACS).
Initial evaluation should include a thorough history and physical examination. The clinician should assess the patient's risk factors for coronary artery disease (CAD), the characteristics of the pain, and accompanying symptoms. Electrocardiogram (ECG) and cardiac biomarkers are essential in the early detection of myocardial ischemia.
When initial investigations are inconclusive, diagnostic imaging can provide further insights. Coronary computed tomography angiography (CCTA) is increasingly used in the evaluation of ACP due to its high negative predictive value. However, it should be used judiciously considering its associated radiation exposure.
Clinical decision rules such as the HEART score can guide clinicians in risk stratification and decision-making. These tools incorporate elements of history, ECG findings, age, risk factors, and troponin levels to predict the likelihood of major adverse cardiac events.
The management of ACP is guided by its underlying cause. In cases of suspected ACS, immediate reperfusion therapy is indicated. For non-cardiac chest pain, appropriate management includes symptom control and addressing the underlying cause.
In conclusion, the evaluation of ACP requires a systematic approach, incorporating clinical history, physical examination, diagnostic investigations, and risk stratification tools. Prompt identification and management of the underlying cause are essential to improve patient outcomes. As clinicians, maintaining a high index of suspicion and utilizing available diagnostic tools judiciously can aid in deciphering the enigma of acute chest pain.
