Anemia, a condition marked by a decrease in the number or functionality of red blood cells, poses a diagnostic challenge due to its multifaceted etiology. This guide aims to streamline the diagnostic approach, enhancing the understanding of this complex condition.
The initial evaluation should involve a thorough history, physical examination, and basic laboratory tests. The history should focus on dietary habits, family history, and any symptoms of bleeding. The physical examination can provide clues such as pallor, jaundice, or splenomegaly. Basic laboratory tests include complete blood count (CBC), reticulocyte count, and peripheral smear.
Anemia is classified based on the mean corpuscular volume (MCV). Microcytic anemia (MCV<80 fL) is often due to iron deficiency or thalassemia. Normocytic anemia (MCV 80-100 fL) can be caused by chronic disease, acute blood loss, or hemolysis. Macrocytic anemia (MCV>100 fL) is typically due to vitamin B12 or folate deficiency, myelodysplastic syndrome, or alcoholism.
Further investigations depend on the initial findings. Iron studies, hemoglobin electrophoresis, or bone marrow examination may be required. For macrocytic anemia, serum B12, folate levels, and a peripheral smear to look for hypersegmented neutrophils are recommended.
In elderly patients, anemia could be a sign of underlying malignancy, necessitating further investigations. In patients with chronic kidney disease, erythropoietin deficiency is a common cause. Also, remember that physiological anemia of pregnancy is a common finding and does not always require treatment.
Anemia's diverse etiology necessitates a systematic approach to diagnosis. By methodically evaluating the patient's history, physical examination, and laboratory findings, physicians can diagnose and manage this common but complex condition more effectively.
