Unexplained weight loss (UWL) is a frequently encountered clinical problem that poses diagnostic challenges for healthcare professionals. It often indicates a serious underlying condition and requires a thorough, systematic approach for evaluation.
Initial evaluation should focus on the patient's history and physical examination. It's essential to ascertain the duration, amount of weight loss, and associated symptoms. Important elements include dietary history, medication use, psychosocial history, and family history of malignancy.
Basic laboratory tests are recommended for all patients with UWL. These include complete blood count, metabolic panel, liver function tests, thyroid function tests, HIV testing, and urinalysis. Further investigations such as imaging studies or endoscopy should be guided by clinical suspicion based on initial evaluation.
Management of UWL should be directed towards treating the underlying cause. In cases where no specific cause is identified, empirical treatment with nutritional supplementation and follow-up for re-evaluation are recommended. Psychosocial interventions may be required in cases of stress or depression-induced weight loss.
Evaluating unexplained weight loss necessitates a comprehensive, systematic approach that begins with a thorough history and physical examination, followed by appropriate diagnostic testing. Management should focus on addressing the underlying cause. Continuous re-evaluation is crucial as new symptoms may emerge over time, leading to a definitive diagnosis. This approach ensures optimal patient care while minimizing unnecessary investigations.
