Dizziness, a common complaint in clinical practice, often poses a diagnostic challenge due to its multifactorial nature. Vertigo, a subtype of dizziness, is characterized by a sensation of spinning or motion. This article aims to provide a comprehensive clinical approach to diagnosing and managing vertigo.
Vertigo is often a symptom of a dysfunction in the vestibular system. It can be categorized into peripheral and central vertigo. Peripheral vertigo is more common and usually associated with benign conditions like benign paroxysmal positional vertigo (BPPV), Ménière's disease, and vestibular neuritis. Central vertigo, on the other hand, may indicate serious conditions such as stroke or multiple sclerosis.
History taking and physical examination remain the cornerstone of vertigo diagnosis. The timing, triggers, and associated symptoms can help differentiate between peripheral and central causes. For instance, episodic vertigo provoked by positional changes is suggestive of BPPV. In contrast, vertigo accompanied by other neurological symptoms may indicate a central cause. Diagnostic tests like electronystagmography (ENG) or videonystagmography (VNG) can be useful in certain cases.
Management of vertigo is tailored according to the underlying cause. BPPV can often be treated with canalith repositioning maneuvers. Vestibular suppressant medications can provide temporary relief but are not recommended for long-term use due to potential side effects. In cases of central vertigo, treating the underlying neurological condition is crucial. Vestibular rehabilitation therapy can also be beneficial for persistent vertigo.
Vertigo is a complex symptom that requires a meticulous clinical approach for accurate diagnosis and effective management. Understanding the pathophysiology, recognizing the clinical features, and being familiar with the appropriate diagnostic tests and treatment modalities are essential for optimal patient care.
