Vertigo is primarily a vestibular system disorder, with causes ranging from benign paroxysmal positional vertigo (BPPV) to more serious conditions like Meniere's disease or vestibular neuritis. It can also be a secondary symptom of neurological disorders such as stroke or multiple sclerosis. It's essential to distinguish vertigo from other forms of dizziness, as the management strategies differ significantly.
Diagnosis of vertigo involves a detailed patient history, physical examination, and, when necessary, specific diagnostic tests. The Dix-Hallpike maneuver is a standard test for BPPV, while caloric testing and vestibular evoked myogenic potentials (VEMP) can help diagnose other vestibular disorders. Management of vertigo is multifaceted, involving symptomatic treatment, physical therapy, and addressing the underlying cause. Medications such as meclizine and diazepam can provide symptomatic relief. Canalith repositioning maneuvers are effective for BPPV, while vestibular rehabilitation therapy can help in chronic vertigo. In refractory cases, surgical interventions may be considered.
