Chronic cough, defined as a cough lasting more than eight weeks, poses a significant challenge to healthcare professionals. Its multifactorial etiology, combined with the subjective nature of symptoms, often complicates diagnosis and treatment.
Common causes of chronic cough include gastroesophageal reflux disease (GERD), asthma, and upper airway cough syndrome (UACS) secondary to rhinosinus conditions. However, non-acid reflux, non-asthmatic eosinophilic bronchitis, and atypical presentations of lung disease should also be considered. A comprehensive patient history, physical examination, and targeted diagnostic testing are crucial in identifying the underlying cause.
Management of chronic cough should be tailored to the identified cause. Empiric treatment for GERD, UACS, and asthma may be initiated in the absence of definitive diagnostic testing. However, refractory cases require further investigation. Non-pharmacological interventions, including lifestyle modifications and speech therapy, have proven beneficial in certain cases. Pharmacological treatments, such as inhaled corticosteroids, proton pump inhibitors, and gabapentin, have demonstrated efficacy in randomized controlled trials.
Novel therapies targeting neuronal hypersensitivity, a common feature in chronic cough, are under investigation. P2X3 receptor antagonists, such as gefapixant, have shown promising results in phase II clinical trials, offering a potential new avenue for treatment.
Chronic cough management requires a comprehensive, patient-centered approach. Understanding the multifactorial etiology and adopting a systematic diagnostic and therapeutic strategy can improve patient outcomes. Emerging therapies targeting neuronal hypersensitivity offer exciting future possibilities. Continued research and clinical collaboration are paramount in further unraveling the complexities of this common, yet challenging, clinical entity.
