Vertigo, a common clinical symptom, often presents a diagnostic challenge due to its multifaceted nature. This symptom, characterized by a sensation of self-motion or environmental motion, is frequently a manifestation of vestibular disorders. Understanding its pathophysiology and clinical presentation can enhance diagnostic accuracy and patient management.
Vertigo is primarily a symptom of vestibular dysfunction, resulting from diverse etiologies such as benign paroxysmal positional vertigo (BPPV), Meniere's disease, or vestibular neuritis. It can also occur secondary to central nervous system disorders like stroke or multiple sclerosis. A careful history and physical examination are critical in differentiating peripheral from central causes of vertigo.
When evaluating a patient with vertigo, a systematic approach is crucial. The patient's history should focus on the onset, duration, and triggers of vertigo. Physical examination should include a detailed neurological and otoscopic assessment. Special attention should be given to nystagmus, head impulse test, and Romberg's test. Audiometric testing and neuroimaging may also be necessary for definitive diagnosis.
Management of vertigo depends on the underlying cause. BPPV, the most common cause of vertigo, is typically managed with canalith repositioning maneuvers. Meniere's disease may require dietary modifications, diuretics, and possibly surgery. Vestibular neuritis often responds to corticosteroids and vestibular rehabilitation. In cases of central vertigo, the underlying neurological condition must be addressed.
Vertigo is a common yet complex clinical symptom requiring a comprehensive and systematic approach for accurate diagnosis and effective management. By understanding the pathophysiology and adopting a thorough clinical evaluation strategy, healthcare professionals can enhance patient outcomes and reduce the burden of this disabling condition.
