Cardiovascular disease (CVD) remains a leading cause of morbidity and mortality among patients with diabetes. Healthcare professionals must be adept at identifying and mitigating cardiovascular risks in this vulnerable population.
Cardiovascular risk assessment in diabetic patients is multifaceted. It involves evaluating traditional risk factors such as hypertension, dyslipidemia, and smoking, alongside monitoring glycemic control. Use of risk prediction models, such as the Framingham Risk Score, can aid in quantifying cardiovascular risk.
Hyperglycemia is a significant contributor to cardiovascular risk in diabetes. Achieving and maintaining optimal glycemic control is essential. Individualized HbA1c targets should be set, with most patients aiming for levels below 7%.
Interventions to address traditional risk factors are key. Blood pressure should be controlled to below 130/80 mmHg in most patients. Statin therapy is recommended for all diabetic patients over 40 years, and for younger patients with additional risk factors. Smoking cessation programs should be offered where relevant.
Emerging evidence suggests certain antidiabetic medications, including SGLT-2 inhibitors and GLP-1 receptor agonists, may confer additional cardiovascular benefits. Consideration should be given to their use in patients with established CVD or multiple risk factors.
Lifestyle modifications, including dietary changes and regular physical activity, are cornerstones of cardiovascular risk reduction. Healthcare professionals should provide ongoing support and education to help patients adopt healthier habits.
Cardiovascular risk management in diabetic patients requires a comprehensive, individualized approach. By addressing glycemic control, traditional risk factors, and lifestyle habits, and by considering the use of newer therapies, healthcare professionals can significantly mitigate cardiovascular risk in this population.
