Chronic cough, persisting for over eight weeks, is a common yet challenging condition encountered in clinical practice. Despite its prevalence, the multifactorial nature often leads to diagnostic uncertainty and therapeutic complexity.
Initial diagnostic efforts should focus on identifying common causes such as gastroesophageal reflux disease (GERD), asthma, and upper airway cough syndrome (UACS) due to rhinosinus conditions. If these are excluded, consider less common etiologies, including non-asthmatic eosinophilic bronchitis, chronic bronchitis, and bronchiectasis. A systematic approach, including a detailed history, physical examination, and targeted investigations, remains paramount.
High-resolution computed tomography (HRCT) can reveal bronchiectasis, interstitial lung disease, or malignancies, while Pulmonary Function Tests (PFTs) can uncover obstructive or restrictive abnormalities. However, their routine use is not recommended, and they should be reserved for cases with unexplained symptoms or abnormal initial investigations.
Treatment should be etiology-specific and may involve lifestyle modifications, pharmacological interventions, or rarely, surgery. Empirical therapy for GERD, UACS, or asthma can be considered if the diagnosis is uncertain. Refractory cases may benefit from referral to a cough specialist or a multidisciplinary team.
Emerging research focuses on understanding the pathophysiology of chronic cough, including the role of neuronal hypersensitivity and novel therapeutic targets. However, the translation of these findings into clinical practice requires further studies.
Chronic cough poses diagnostic and therapeutic challenges in medical practice. A comprehensive and systematic approach, coupled with an understanding of common and uncommon causes, is essential for effective management. Future research promises to enhance our understanding and treatment of this complex condition.
