Chronic cough, defined as a cough persisting for more than eight weeks, is a common clinical problem encountered in primary care and respiratory specialist settings. Despite its prevalence, it remains a challenging condition to diagnose and manage effectively.
Chronic cough can be attributed to a myriad of causes, often categorized into three broad groups: respiratory conditions, non-respiratory conditions, and idiopathic causes. The most common respiratory conditions include chronic obstructive pulmonary disease (COPD), asthma, and bronchiectasis. Non-respiratory conditions include gastroesophageal reflux disease (GERD) and postnasal drip syndrome. Idiopathic chronic cough remains a diagnosis of exclusion.
A comprehensive history and physical examination are crucial in the evaluation of chronic cough. Specific attention should be paid to the presence of 'red flag' symptoms such as hemoptysis, unintentional weight loss, and night sweats, which may indicate a more serious underlying condition. Diagnostic tests, including spirometry, chest X-ray, and upper gastrointestinal endoscopy, may be warranted based on clinical suspicion.
Management of chronic cough should be tailored to the underlying cause. Empiric treatment trials for asthma, GERD, and rhinosinusitis can be considered in the absence of diagnostic clarity. In cases of idiopathic chronic cough, non-pharmacological approaches such as speech therapy and behavioral modification have shown promising results.
Recent research has highlighted the role of neuronal hypersensitivity and neurogenic inflammation in chronic cough. New therapeutic agents targeting these pathways are currently under investigation and may provide additional treatment options in the future.
Chronic cough is a complex clinical entity requiring a comprehensive approach for effective management. Further research into its pathophysiology may pave the way for innovative therapeutic strategies, improving outcomes for patients afflicted with this challenging condition.
