Abdominal pain is one of the most common complaints in outpatient and emergency settings, yet it remains a diagnostic challenge for clinicians due to its diverse etiologies. A systematic approach is essential to ensure accurate diagnosis and appropriate treatment.
A comprehensive evaluation strategy begins with a thorough history, including the nature, location, and duration of the pain, associated symptoms, and precipitating factors. This is followed by a detailed physical examination, which may provide clues to the underlying cause.
Diagnostic tools such as laboratory tests, imaging studies, and endoscopic procedures can be invaluable. However, their use should be guided by the clinical suspicion based on the history and physical examination. For instance, a CT scan may be warranted in a patient with suspected appendicitis, while an upper endoscopy may be more appropriate for a patient with epigastric pain and dyspepsia.
Common causes of abdominal pain include gastroenteritis, appendicitis, cholecystitis, peptic ulcer disease, and diverticulitis. Less common but important causes to consider include ischemic bowel disease, abdominal aortic aneurysm, and malignancy. The differential diagnosis can be broadened or narrowed based on the patient's age, sex, and comorbidities.
Management should be tailored to the underlying cause. This may range from conservative measures such as dietary modification and analgesics for functional disorders, to surgical intervention for conditions such as appendicitis or cholecystitis. In some cases, referral to a specialist may be necessary.
Abdominal pain is a complex clinical symptom with a wide range of potential causes. A systematic approach to evaluation, guided by a thorough history and physical examination, can help clinicians navigate this diagnostic enigma. This, combined with judicious use of diagnostic tools and a nuanced understanding of common and uncommon causes, can facilitate effective management and improve patient outcomes.
