Anemia can be classified according to the size of red blood cells (microcytic, normocytic, or macrocytic) or based on the underlying cause (iron deficiency, vitamin B12/folate deficiency, hemolytic, or anemia of chronic disease). This initial classification can guide further diagnostic investigations.
Complete blood count (CBC) and reticulocyte count are the initial investigations for suspected anemia. Further, serum iron studies, vitamin B12 and folate levels, haptoglobin, lactate dehydrogenase (LDH), and peripheral blood smear can provide insights into the underlying etiology. In some cases, bone marrow examination may be warranted.
Interpretation of laboratory results should be done in a systematic manner. Microcytic anemia often suggests iron deficiency or thalassemia, while macrocytic anemia may indicate vitamin B12 or folate deficiency, or myelodysplastic syndrome. Normocytic anemia can be due to anemia of chronic disease, acute blood loss, or hemolysis. Hemolytic anemia can be diagnosed by increased reticulocyte count, LDH, and decreased haptoglobin.
While laboratory investigations are crucial, they should be interpreted in the context of the patient's clinical presentation and history. For instance, dietary history, medication use, family history, and associated symptoms can provide valuable clues to the underlying cause of anemia.
In conclusion, the diagnosis of anemia requires a comprehensive approach that includes etiological classification, appropriate laboratory investigations, and careful interpretation of results in the context of the patient's clinical picture. This approach will enable healthcare professionals to accurately diagnose and manage this common but complex disorder.
