Chronic cough, defined as a cough lasting more than eight weeks, is a common and often challenging condition encountered in clinical practice. Despite its prevalence, a significant number of patients remain undiagnosed or inadequately treated, leading to a substantial impact on quality of life and healthcare resources.
Understanding the pathophysiology of chronic cough is key to formulating a comprehensive diagnostic approach. The most common causes include upper airway cough syndrome (UACS), gastroesophageal reflux disease (GERD), and non-asthmatic eosinophilic bronchitis (NAEB). However, it is crucial to consider less common causes such as lung cancer, tuberculosis, and interstitial lung diseases, particularly in patients with risk factors or atypical presentations.
Diagnostic testing should be individualized based on the patient's clinical history and physical examination. High-resolution computed tomography (HRCT) can be useful in evaluating for structural lung diseases. Meanwhile, pulmonary function tests can help identify airflow obstruction or restriction. In patients with suspected GERD or UACS, pH monitoring or sinus imaging may be warranted.
Treatment should be targeted to the underlying cause. For instance, proton pump inhibitors may be effective for GERD-induced cough, while inhaled corticosteroids can be beneficial for NAEB. However, in cases where a specific cause is not identified, empirical treatment with cough suppressants, neuromodulators, or physical therapy may be considered.
Chronic cough remains a complex clinical problem requiring a comprehensive and individualized approach to diagnosis and treatment. Future research should focus on developing more effective diagnostic tools and treatment strategies, as well as understanding the pathophysiology of refractory chronic cough. With a systematic approach, clinicians can improve the management of this common, yet often challenging condition.
