Gastroesophageal reflux disease (GERD) is a prevalent digestive disorder, affecting a significant portion of the global population. It is characterized by chronic symptoms of mucosal damage produced by stomach acid coming up from the stomach into the esophagus. This article provides a comprehensive guide on the diagnosis and treatment of GERD for healthcare professionals.
Diagnosing GERD involves a thorough patient history and physical examination. Typical symptoms include heartburn, regurgitation, and dysphagia. However, atypical symptoms such as chronic cough, laryngitis, or dental erosions may also be present. Diagnostic tests include upper endoscopy, esophageal manometry, ambulatory pH or impedance-pH monitoring, and barium swallow radiograph. An endoscopy is recommended if alarm symptoms like dysphagia, odynophagia, weight loss, or gastrointestinal bleeding are present.
Initial treatment for GERD involves lifestyle modifications and over-the-counter medications. Lifestyle modifications include weight loss, elevating the head of the bed, and avoiding food triggers. Pharmacological therapy involves the use of antacids, H2-receptor antagonists (H2RAs), and proton pump inhibitors (PPIs). PPIs are the most effective in healing erosive esophagitis and managing GERD symptoms.
For patients unresponsive to initial treatment, a higher dose PPI, a PPI with a different timing, or a PPI twice daily may be considered. Anti-reflux surgery is an option for patients with well-documented reflux who have failed medical therapy or have complications from GERD such as strictures or Barrett's esophagus.
GERD is a common condition that requires a comprehensive approach for effective diagnosis and treatment. Healthcare professionals need to be aware of both typical and atypical symptoms, and the range of diagnostic tests available. Initial treatment involves lifestyle modifications and medication, with surgery as a last resort for refractory cases. Continued research and education are necessary to improve patient outcomes and quality of life.
