Vertigo is typically a manifestation of vestibular dysfunction. It is crucial to distinguish vertigo from other forms of dizziness such as presyncope, disequilibrium, and non-specific dizziness. A thorough patient history and physical examination are the cornerstones of diagnosis.
Key aspects of the history include onset, duration, triggers, associated symptoms, and any previous episodes. A targeted neurological and otological examination, along with specific tests like the Dix-Hallpike maneuver or the head impulse test, can help localize the pathology to the peripheral or central vestibular system.
While the diagnosis of vertigo is primarily clinical, adjunctive diagnostic modalities like vestibular function tests, audiometry, and neuroimaging can provide valuable information. These should be used judiciously based on clinical suspicion and to rule out serious causes like stroke or tumours.
Treatment of vertigo is multifaceted and includes symptomatic relief with medications, vestibular rehabilitation, and in some cases, surgery. The choice of treatment is guided by the underlying cause, severity of symptoms, and the patient's overall health status.
Vertigo, while common, can be daunting to diagnose and manage. A systematic approach to history taking, examination, and judicious use of diagnostic modalities can help clinicians navigate this complex condition. Tailored treatment strategies can significantly improve patient outcomes and quality of life.
