The escalating threat of antimicrobial resistance necessitates judicious antibiotic use in clinical practice. This article provides a comprehensive guide to rational antibiotic use, aiming to promote patient health while mitigating the risk of resistance.
Antibiotic stewardship programs (ASPs) are pivotal in ensuring rational antibiotic use. These programs encourage the selection of optimal antibiotic drug regimens including dosing, duration of therapy, and route of administration. The goal is to maximize clinical outcomes while minimizing unintended consequences of antibiotic use, such as toxicity, the selection of pathogenic organisms, and the emergence of resistance.
Accurate diagnosis is key to rational antibiotic use. Misdiagnosis often leads to unnecessary antibiotic prescriptions. Advancements in diagnostic technology, such as rapid molecular tests, can help in the precise identification of pathogens, thereby guiding appropriate antibiotic therapy.
The duration of antibiotic therapy should be as short as possible to achieve the desired therapeutic outcome. Prolonged antibiotic use increases the risk of resistance. De-escalation, or shifting from broad-spectrum to narrow-spectrum antibiotics when pathogen information is available, is another strategy to minimize resistance.

By implementing robust ASPs, improving diagnostic precision, optimizing antibiotic duration and de-escalation, and fostering education and communication, we can promote patient health while combating the global threat of antibiotic resistance.
