Pulmonary hypertension (PH) is a complex and often misunderstood disease. It is characterized by elevated pulmonary artery pressure and pulmonary vascular resistance, leading to right ventricular failure. This article aims to provide an in-depth understanding of the fundamentals of PH.
PH is a pathophysiological disorder that involves multiple pathways. These include vasoconstriction, vascular remodeling, thrombosis, and inflammation. The endothelial dysfunction leads to an imbalance between vasodilators and vasoconstrictors, contributing to increased pulmonary vascular resistance.
PH often presents insidiously, with nonspecific symptoms such as dyspnea on exertion, fatigue, and syncope. As the disease progresses, right ventricular failure symptoms such as edema and ascites may become apparent. Early diagnosis is crucial to improve patient outcomes.
Diagnosis of PH involves a combination of clinical assessment, echocardiography, and right heart catheterization. An elevated mean pulmonary artery pressure of >20mmHg at rest, as measured by right heart catheterization, is diagnostic of PH. Echocardiography serves as a non-invasive screening tool, while right heart catheterization confirms the diagnosis.
Management of PH involves addressing the underlying cause, symptom control, and slowing disease progression. Therapies include vasodilator drugs, anticoagulants, diuretics, and in severe cases, lung transplantation. A multidisciplinary approach is essential for optimal patient care.
Understanding the fundamentals of PH is vital for early diagnosis and effective management. It is a complex disease that requires a comprehensive diagnostic approach and a multidisciplinary management strategy. By increasing our understanding of PH, we can improve patient outcomes and quality of life.
