Parkinson's disease (PD) is a chronic, progressive neurodegenerative disorder that primarily affects movement. Characterized by the degeneration of dopaminergic neurons in the substantia nigra, PD presents a complex clinical challenge necessitating a comprehensive understanding by healthcare professionals.
While the exact cause of PD remains unknown, it is thought to result from a combination of genetic and environmental factors. Pathophysiologically, the loss of dopaminergic neurons leads to an imbalance in the basal ganglia's neurotransmitter levels, resulting in the motor symptoms of PD.
PD typically manifests with bradykinesia, rigidity, and resting tremor. Non-motor symptoms such as autonomic dysfunction, cognitive impairment, and mood disorders may also be present. The disease is progressive, with symptoms worsening over time.
Diagnosis of PD is primarily clinical, based on the presence of cardinal motor symptoms and response to dopaminergic therapy. Neuroimaging can support the diagnosis, but it is not routinely used due to its limited sensitivity and specificity.
Management of PD involves pharmacological therapy to enhance dopaminergic function, alongside non-pharmacological interventions such as physical therapy. Deep brain stimulation may be considered in advanced cases.
In conclusion, Parkinson's disease is a complex neurodegenerative disorder that requires a comprehensive and multidisciplinary approach to management. As healthcare professionals, it is essential to stay updated on the latest advancements in PD research to provide optimal care to our patients.
