Introduction

Pulmonary Function Testing (PFT) is an integral part of respiratory disease diagnosis and management. However, understanding and interpreting these tests can be challenging due to their complexity. This guide aims to elucidate the intricacies of PFT interpretation for healthcare professionals.

Understanding PFT Components

Key components of PFT include spirometry, lung volumes, and diffusion capacity. Spirometry measures the speed of air movement in the lungs, providing information on airway obstruction. Lung volumes assess the amount of air in different phases of the respiratory cycle, revealing restrictive patterns. Diffusion capacity evaluates how well oxygen moves from the lungs into the blood.

Interpreting Spirometry

Spirometry interpretation involves three key parameters: Forced Vital Capacity (FVC), Forced Expiratory Volume in one second (FEV1), and the FEV1/FVC ratio. A reduced FEV1/FVC ratio suggests obstructive disease, while a reduced FVC with a normal or increased FEV1/FVC ratio indicates restrictive disease.

Assessing Lung Volumes

Restrictive diseases reduce all lung volumes, while obstructive diseases may increase residual volume and total lung capacity. Key parameters include Total Lung Capacity (TLC), Vital Capacity (VC), and Residual Volume (RV). A reduced VC or TLC suggests restrictive disease, and an increased RV/TLC ratio suggests air trapping typical of obstructive disease.

Evaluating Diffusion Capacity

The Diffusion Capacity of the Lung for Carbon Monoxide (DLCO) measures gas transfer across the alveolar-capillary membrane. Reduced DLCO can indicate interstitial lung disease, emphysema, or pulmonary vascular disease.

Conclusion

Interpreting PFTs is a complex task requiring a strong understanding of respiratory physiology and pathology. However, with a systematic approach to each component of the PFT and a clear understanding of the disease patterns they represent, healthcare professionals can effectively use these vital tools to diagnose and manage respiratory diseases.
