Antimicrobial resistance is a global health crisis that threatens the effective prevention and treatment of an ever-increasing range of infections. Antimicrobial stewardship programs (ASPs) have been identified as a key strategy in combating this crisis. This article discusses a strategic approach to implementing effective ASPs in hospital-based healthcare settings.
ASPs are coordinated interventions designed to improve and measure the appropriate use of antimicrobials by promoting the selection of the optimal antimicrobial drug regimen. They aim to reduce microbial resistance, decrease the spread of infections caused by multidrug-resistant organisms, and improve patient outcomes.
An effective ASP requires a multidisciplinary team approach. This team should include an infectious disease physician, a clinical pharmacist with infectious disease training, clinical microbiologists, information system specialists, and infection control professionals. Collaboration and communication amongst these professionals are pivotal to the success of the ASP.
Core strategies for successful ASPs include prospective audit with intervention and feedback, formulary restriction and preauthorization, education, and guidelines and clinical pathways. These strategies should be tailored to the specific needs and resources of the hospital to ensure effectiveness.
Monitoring the use of antimicrobials and the results of ASPs is essential. Regular reporting of antimicrobial use and resistance trends to healthcare providers, pharmacists, and hospital administration can facilitate the ongoing success of the program.
In conclusion, implementing effective ASPs in hospital-based healthcare settings is a strategic approach in the fight against antimicrobial resistance. By understanding the need for ASPs, creating a multidisciplinary team, implementing core strategies, and monitoring and reporting, we can optimize antimicrobial use, improve patient outcomes, and combat the global crisis of antimicrobial resistance.
