History taking is paramount in the evaluation of abdominal pain. It is crucial to ascertain the onset, location, character, radiation, associated symptoms, and relieving and exacerbating factors. A thorough review of systems, including genitourinary, gastrointestinal, and gynecological history in females, is essential. Past medical, surgical, and family history can also provide valuable clues to the diagnosis.
A systematic physical examination should follow a detailed history. Inspection, palpation, percussion, and auscultation of the abdomen can reveal signs of an underlying pathology. The presence of scars may indicate previous surgeries, while the nature of the pain can help differentiate between visceral, parietal, and referred pain. Examination of the genitourinary system and a digital rectal examination may be required in certain cases.
Investigations should be guided by the history and physical examination. Basic investigations include complete blood count, liver function tests, renal function tests, electrolytes, and imaging modalities such as ultrasound or computed tomography. More specific investigations like endoscopy, colonoscopy, or laparoscopy may be required based on the clinical suspicion.
The differential diagnosis of abdominal pain is broad, encompassing conditions such as appendicitis, cholecystitis, pancreatitis, peptic ulcer disease, and gynecological disorders in females. It is essential to consider life-threatening conditions such as aortic aneurysm and myocardial infarction, which can present with abdominal pain.
Abdominal pain is a complex clinical entity, with a vast array of potential etiologies. A structured approach to history taking, physical examination, and investigations can aid in narrowing down the differential diagnosis and guide appropriate management. Continual education and practice are crucial for mastering this challenging aspect of clinical practice.
