Dyspnea, or breathlessness, is a common symptom encountered in clinical practice. Its subjective nature and multifactorial etiology can make it a challenging symptom to evaluate and manage. Initial evaluation of dyspnea should involve a detailed history and physical examination. Important factors to consider include onset, duration, severity, precipitating factors, and associated symptoms. Objective measures such as pulse oximetry, spirometry, and peak flow rate can provide useful information about the patient’s respiratory status.
The differential diagnosis for dyspnea is broad and includes cardiac, pulmonary, and non-physiological causes. Cardiac causes include heart failure and ischemic heart disease. Pulmonary causes include chronic obstructive pulmonary disease (COPD), asthma, pneumonia, and pulmonary embolism. Non-physiological causes include anxiety and deconditioning. Careful consideration of the patient’s history, physical examination, and diagnostic testing can help narrow the differential diagnosis.
Management of dyspnea should be tailored to the underlying cause. For cardiac causes, management may involve medications, lifestyle modifications, and potentially invasive procedures. Pulmonary causes may require bronchodilators, corticosteroids, antibiotics, or anticoagulants, depending on the specific diagnosis. Non-physiological causes may benefit from behavioral therapies, physical rehabilitation, or treatment of underlying psychiatric conditions.
Dyspnea is a common but complex symptom that requires a systematic and thorough clinical approach. By understanding the potential etiologies of dyspnea and utilizing a methodical approach to assessment and management, healthcare professionals can effectively diagnose and treat this distressing symptom, improving patient outcomes and quality of life.
