Abstract

Reversible autoimmune encephalopathy (RAE) represents a spectrum of neurologic disorders characterized by subacute onset neuropsychiatric symptoms and evidence of central nervous system inflammation, with a notable response to immunotherapy. This review synthesizes current evidence regarding the epidemiology, pathophysiology, risk factors, clinical manifestations, diagnostic approaches, treatment modalities, emerging therapies, and guideline-driven recommendations relevant to RAE. Emphasis is placed on the importance of early recognition, mechanism-based understanding, and practical management strategies to improve patient outcomes. Introduction

Reversible autoimmune encephalopathy is a critical, yet often under-recognized, subset of central nervous system disorders. It encompasses conditions like steroid-responsive encephalopathy associated with autoimmune thyroiditis (SREAT), anti-NMDA receptor encephalitis, and other antibody-mediated syndromes. The clinical challenge lies in distinguishing RAE from infectious, metabolic, and primary psychiatric etiologies, as early intervention can be transformative. Recent advances in autoantibody detection and a growing understanding of neuroimmune interactions have redefined the diagnostic and therapeutic landscape, necessitating heightened clinical awareness and evidence-based management.

Epidemiology / Disease Burden

RAE is increasingly recognized due to improved clinical vigilance and advanced antibody testing. Epidemiological data suggest that autoimmune encephalitides collectively have an annual incidence of roughly 1.2–1.5 per 100,000 individuals, with anti-NMDA receptor encephalitis comprising a significant subset. Pathophysiology

The pathogenesis of RAE involves aberrant immune responses targeting neuronal cell surface or synaptic antigens, resulting in CNS inflammation and neuronal dysfunction. In SREAT, high titers of anti-thyroid peroxidase or anti-thyroglobulin antibodies are common, although their direct pathogenicity remains debated. In contrast, anti-NMDA receptor encephalitis is mediated by antibodies targeting the GluN1 subunit, leading to receptor internalization and synaptic dysfunction. Cellular and humoral immune mechanisms, including T-cell infiltration and cytokine production, play contributory roles. Blood-brain barrier disruption is often an enabling event, facilitating antibody access to neural tissue.

Risk Factors

Identified risk factors for RAE include female sex, presence of systemic autoimmunity (e.g., thyroiditis, lupus), certain HLA types, malignancies (notably ovarian teratomas in anti-NMDA receptor encephalitis), and preceding infections. Genetic predisposition is suggested by familial clustering in some cases. Environmental triggers, molecular mimicry post-infection, and immune dysregulation are suspected contributors. Awareness of these risk factors can guide early suspicion and prompt investigation in at-risk individuals presenting with neuropsychiatric symptoms.

Clinical Features

RAE presents with a spectrum of neuropsychiatric manifestations, often evolving subacutely over days to weeks. Common features include altered mental status, cognitive impairment, behavioral changes, psychosis, seizures, movement disorders, language dysfunction, and autonomic instability. SREAT may be heralded by confusion, myoclonus, and stroke-like episodes. Anti-NMDA receptor encephalitis classically progresses from psychiatric symptoms to seizures, dyskinesias, and autonomic dysfunction. Fluctuating symptoms and responsiveness to immunotherapy are distinguishing features. Ancillary signs may include headache, sleep disturbances, and focal neurologic deficits.

Diagnosis

Diagnosis relies on integrating clinical suspicion with supportive laboratory and neuroimaging findings. Core investigations include serum and cerebrospinal fluid (CSF) analysis for autoantibodies (e.g., anti-NMDAR, anti-LGI1, anti-thyroid antibodies), inflammatory markers, and exclusion of infectious etiologies. MRI is frequently abnormal, revealing T2/FLAIR hyperintensities in limbic or cortical regions, though findings may be subtle or absent. EEG often demonstrates nonspecific slowing or epileptiform activity. Screening for underlying malignancy is essential, particularly in paraneoplastic subtypes. The diagnosis is further supported by a favorable response to immunotherapy, particularly corticosteroids or IVIG.

Treatment & Management

Early initiation of immunotherapy remains the cornerstone of management. First-line therapy typically involves high-dose corticosteroids, intravenous immunoglobulin (IVIG), or plasma exchange. In cases associated with neoplasms, oncologic management is paramount. For refractory or relapsing disease, second-line agents such as rituximab, cyclophosphamide, or mycophenolate mofetil may be employed. Symptomatic management of seizures, psychiatric symptoms, and autonomic instability is vital. A multidisciplinary approach encompassing neurology, psychiatry, immunology, and oncology optimizes patient outcomes. Long-term follow-up is required due to risk of relapse and ongoing neurocognitive sequelae.

Recent Advances / Emerging Therapies

Recent years have seen the emergence of novel therapeutics targeting specific immune pathways. Monoclonal antibodies (e.g., rituximab, tocilizumab) are increasingly employed, particularly in antibody-mediated encephalopathies refractory to conventional immunosuppression. Improved antibody panels have enhanced diagnostic specificity, allowing for stratified therapy. There is growing interest in complement inhibitors and small-molecule agents targeting B-cell signaling. Research into neuroprotective strategies and biomarkers of disease activity is ongoing, aiming to individualize therapy and predict outcomes. The development of international registries and collaborative networks is facilitating large-scale studies to refine evidence-based protocols.

Guideline Recommendations

Consensus guidelines from organizations such as the American Academy of Neurology and the European Federation of Neurological Societies advocate for a high index of suspicion in cases of subacute neuropsychiatric decline, early antibody testing, prompt initiation of immunotherapy, and comprehensive malignancy screening. Multimodal diagnostic pathways are recommended, integrating clinical, serological, and imaging data. Maintenance immunosuppression may be considered in relapsing cases. Rehabilitation and neuropsychological support are emphasized as integral to long-term recovery. Regular re-evaluation is advised to monitor for relapse or evolving malignancy.

Reversible autoimmune encephalopathy is a clinically heterogeneous yet potentially treatable group of disorders. Early recognition, mechanism-based diagnosis, and prompt immunotherapy are critical to favorable outcomes. Ongoing research is expanding our understanding of immune-mediated CNS disease and informing the development of targeted therapies. Clinicians should maintain a high level of suspicion for RAE in patients with unexplained neuropsychiatric symptoms, and adhere to emerging guidelines to optimize patient care.
