The management of complex community-based illnesses encompassing conditions such as advanced heart failure, chronic obstructive pulmonary disease (COPD), diabetes with complications, and frailty syndromes poses significant challenges for modern healthcare systems. Traditional linear care models often fail to account for the heterogeneity and unpredictability inherent in these populations.
