This review synthesizes contemporary evidence on epidemiology, pathophysiology, risk factors, clinical assessment, and perioperative interventions, highlighting cutting-edge strategies and future directions for personalized care in female surgical patients.
The growing recognition of sex- and gender-based differences in medicine has catalyzed a paradigm shift towards individualized perioperative care for women. Historically, clinical trials and protocols have predominantly focused on male populations, leading to suboptimal care for female patients. Innovations in perioperative medicine now emphasize the need to address the unique physiological and psychosocial factors influencing surgical outcomes in women, including hormonal fluctuations, reproductive considerations, and higher prevalence of specific comorbidities. This comprehensive review aims to equip clinicians with the latest evidence and recommendations to optimize perioperative management for women across all stages of life.
Women account for a significant proportion of global surgical procedures, with increasing surgical rates in elderly and reproductive-aged populations. Epidemiological data highlight sex-based disparities in perioperative morbidity and mortality, with women experiencing higher rates of certain complications, such as postoperative nausea and vomiting (PONV), thromboembolism, and pain syndromes. Awareness of these clinical features is critical for timely diagnosis and holistic perioperative support.
Diagnostic algorithms in perioperative care must be adapted to recognize sex-specific presentations and risks. Routine preoperative testing should include pregnancy screening for all women of childbearing age, osteoporosis risk assessment for postmenopausal women, and screening for gynecologic malignancies where indicated. Advanced imaging modalities, laboratory markers, and risk prediction tools have improved diagnostic accuracy, but must be interpreted in the context of female physiology. Collaborative, multidisciplinary evaluation incorporating gynecology, anesthesiology, cardiology, and psychology optimizes diagnostic precision and perioperative safety.
Management strategies for women undergoing surgery must be individualized to optimize outcomes. Preoperative optimization includes addressing anemia, nutritional deficiencies, and psychosocial factors. Intraoperative considerations involve dose adjustments for anesthetic agents, thromboprophylaxis tailored to hormonal status, and meticulous fluid management to account for physiological variations. Postoperative care should prioritize multimodal analgesia, early mobilization, and strategies to prevent PONV and thromboembolic events. Enhanced Recovery After Surgery (ERAS) protocols, when adapted to female-specific needs, have demonstrated improvements in recovery times and patient satisfaction.
Recent years have seen the emergence of innovative technologies and therapies to enhance perioperative care in women. Pharmacogenomic profiling enables personalized analgesic and anesthetic regimens, while minimally invasive and robotic-assisted surgical techniques reduce morbidity and facilitate faster recovery in gynecologic and breast surgeries. Hormone modulation therapies, targeted immunonutrition, and wearable monitoring devices are being integrated into perioperative protocols. Digital health platforms, including telemedicine and app-based support, offer new avenues for perioperative education, risk assessment, and remote monitoring particularly beneficial for women in underserved or rural areas.
Leading professional organizations, including the American College of Surgeons, American Society of Anesthesiologists, and specialty societies, now advocate for sex-specific perioperative guidelines. Recommendations include routine pregnancy screening, individualized VTE prophylaxis, consideration of menstrual and menopausal status in surgical timing, and integration of mental health screening into perioperative pathways. ERAS guidelines have been adapted for gynecologic and obstetric surgeries, emphasizing early feeding, opioid-sparing analgesia, and prevention of surgical site infections. Ongoing research and consensus-building are refining these guidelines to further enhance individualized care for women.
Innovations in women's health have transformed the landscape of perioperative care, offering unprecedented opportunities for individualized, evidence-based management across the female lifespan. Clinicians must remain vigilant to the unique physiological, psychological, and social determinants that influence surgical outcomes in women, leveraging emerging technologies and updated guidelines to deliver optimal care. Continued research, interdisciplinary collaboration, and education will be essential to closing sex-based gaps in perioperative medicine and ensuring equitable, high-quality care for all female patients.
