Non-Alcoholic Fatty Liver Disease (NAFLD) is a growing concern in the medical fraternity due to its increasing prevalence and potential to progress to severe liver diseases. This article aims to provide an overview of NAFLD, its pathogenesis, and the current strategies for clinical management.
NAFLD is now the most common chronic liver disease worldwide, affecting about 25% of the global population. It is closely associated with metabolic syndrome, obesity, and type 2 diabetes. The prevalence is expected to rise with the increasing incidence of these conditions.
NAFLD is characterized by excess fat accumulation in the liver, not caused by alcohol consumption. The pathogenesis of NAFLD is multifactorial, involving insulin resistance, oxidative stress, and an inflammatory response. It is often described by the 'two-hit' theory. The 'first hit' leads to hepatic steatosis, while the 'second hit' involves inflammation and fibrosis, leading to non-alcoholic steatohepatitis (NASH), which can progress to cirrhosis.
The management of NAFLD involves lifestyle modifications, pharmacological interventions, and in severe cases, liver transplantation. Lifestyle modifications, including weight loss, diet change, and increased physical activity, are the first-line treatment. Pharmacological options are limited and mainly target comorbid conditions like diabetes and dyslipidemia. Liver transplantation remains the last resort for patients with end-stage liver disease.
NAFLD is a significant public health issue with a complex pathogenesis and limited treatment options. Early detection and intervention are crucial to prevent progression to severe liver diseases. Healthcare professionals need to be aware of the risk factors, pathogenesis, and management strategies for NAFLD to provide optimal care to patients.
