Peptic Ulcer Disease (PUD) remains a prevalent gastrointestinal disorder that presents significant morbidity and mortality worldwide. Recent advancements in diagnosis, management, and prevention strategies have reshaped the clinical approach to PUD.
Endoscopy, the gold standard for PUD diagnosis, has seen advancements in endoscopic ultrasound and confocal laser endomicroscopy. These technologies provide detailed imaging of the ulcer base, allowing for accurate diagnosis and risk stratification.
Proton pump inhibitors (PPIs) remain the cornerstone of PUD treatment. However, novel therapies such as potassium-competitive acid blockers (P-CABs) show promise in offering superior acid suppression. Additionally, the use of H. pylori 'test-and-treat' strategies has significantly reduced recurrence rates.
Preventive strategies focus on modifiable risk factors, including non-steroidal anti-inflammatory drug (NSAID) use and H. pylori infection. Guidelines recommend risk stratification of NSAID users and eradication of H. pylori in infected individuals.
Advancements in diagnostic technologies and management strategies, coupled with effective preventive measures, have improved the clinical outcome of PUD patients. Ongoing research and development promise further enhancements in the approach to PUD, ensuring that healthcare professionals remain equipped to provide the best possible care.
