Introduction

Parkinson’s disease (PD) is a neurodegenerative disorder that primarily affects dopamine-producing neurons in the substantia nigra, a part of the brain that plays a key role in reward and movement. This article aims to provide an in-depth overview of the pathophysiology, diagnosis, and management of PD.

Pathophysiology of Parkinson’s Disease

The hallmark of PD is the progressive loss of dopaminergic neurons in the substantia nigra, leading to a decrease in striatal dopamine levels. This results in the characteristic motor symptoms of PD, including bradykinesia, resting tremor, rigidity, and postural instability. Non-motor symptoms such as cognitive impairment, depression, and autonomic dysfunction may also occur due to the involvement of non-dopaminergic pathways.

Diagnosing Parkinson’s Disease

Diagnosis of PD is primarily clinical, based on the presence of characteristic motor symptoms and their response to dopaminergic therapy. Neuroimaging techniques such as dopamine transporter (DAT) SPECT can support the diagnosis, but are not routinely used due to their high cost and limited availability. There are no specific biomarkers for PD, which can make the diagnosis challenging, particularly in early stages of the disease.

Management of Parkinson’s Disease

Management of PD is symptomatic, aiming to improve quality of life by reducing motor and non-motor symptoms. Pharmacological therapy is the cornerstone of treatment, with levodopa being the most effective drug. Other medications include dopamine agonists, MAO-B inhibitors, and COMT inhibitors. Non-pharmacological interventions, such as physiotherapy and occupational therapy, also play a crucial role in managing the disease.

Conclusion

Parkinson’s disease is a complex neurodegenerative disorder with a multifaceted pathophysiology and clinical presentation. A thorough understanding of its pathophysiology, diagnosis, and management strategies is essential for healthcare professionals to provide optimal care for patients with PD.
