Enteric fever, primarily caused by Salmonella Typhi and Salmonella Paratyphi, remains a significant health challenge, especially in developing countries. This article presents a comprehensive analysis of the current best practices in the management and treatment of this condition.
The causative agents, S. Typhi and S. Paratyphi, are human-specific pathogens transmitted primarily through contaminated food and water. The diagnosis typically involves blood cultures in the first week of illness and bone marrow cultures in the second week. Widal test, though popular, is less specific and sensitive.
Empirical therapy should be initiated as soon as enteric fever is suspected, with the choice of antibiotic guided by local susceptibility patterns. Fluoroquinolones, third-generation cephalosporins, and azithromycin are commonly used. However, increasing drug resistance necessitates regular surveillance and susceptibility testing.
Supportive care is crucial in managing enteric fever. Adequate hydration, correction of electrolyte imbalances, and management of complications like intestinal perforation or encephalopathy are integral to patient care.
Vaccination plays a pivotal role in the prevention of enteric fever. Two vaccines, Vi polysaccharide vaccine (ViPS) and Ty21a, are currently available. Both vaccines are moderately effective and can be used in endemic areas and for travelers to these regions.
Effective management of enteric fever involves timely diagnosis, appropriate antimicrobial therapy, supportive care, and prevention through vaccination. Continuous surveillance of antimicrobial resistance patterns is necessary to guide treatment decisions. Ultimately, improvements in sanitation and access to clean water are vital in reducing the burden of this disease.
