This review outlines the current landscape of drug safety surveillance, emphasizing evidence-based practices, risk assessment, and guideline-driven recommendations. By integrating recent clinical data and mechanistic insights, we aim to equip healthcare professionals with a comprehensive understanding of the surveillance strategies necessary to optimize outcomes and minimize adverse effects during hormonal support in ART.
Assisted reproductive technologies have revolutionized infertility treatment, with hormonal support being indispensable for successful embryonic implantation and pregnancy maintenance. However, these therapies expose patients to exogenous hormones, necessitating a robust framework for drug safety surveillance. Ongoing vigilance is required to detect, assess, and mitigate adverse drug reactions (ADRs) that may compromise maternal and fetal health. This article synthesizes current evidence and guidelines, focusing on the surveillance of hormonal agents during the critical periconceptional and early gestational periods in ART.
Globally, infertility affects approximately 8-12% of reproductive-age couples, with over 2.5 million ART cycles performed annually. Hormonal support, particularly with progesterone, human chorionic gonadotropin (hCG), and gonadotropin-releasing hormone (GnRH) analogs, is integral to these protocols. Despite their efficacy, the widespread utilization of these agents elevates the population at risk for potential ADRs, highlighting the substantial public health relevance of drug safety surveillance in this context.
Hormonal therapies in embryology manipulate the hypothalamic-pituitary-gonadal axis to promote follicular development, ovulation, and endometrial receptivity. Progesterone supports luteal phase and implantation, while exogenous gonadotropins induce multifollicular growth. The supraphysiological hormonal milieu, however, can precipitate adverse events such as ovarian hyperstimulation syndrome (OHSS), thromboembolism, and perturbations in maternal-fetal hormonal signaling. Mechanism-based surveillance focuses on recognizing these predictable, pathway-mediated risks.
Key risk factors for ADRs during hormonal support include high-dose or prolonged hormone exposure, advanced maternal age, polycystic ovary syndrome (PCOS), prior history of OHSS, and comorbid conditions such as thrombophilia. Genetic predispositions, such as factor V Leiden mutations, further amplify risks associated with estrogen-containing regimens. Patient-specific risk stratification is crucial for individualized surveillance and intervention planning.
Adverse reactions range from mild (local injection site reactions, gastrointestinal disturbance) to severe (OHSS, thromboembolic events, hypertensive disorders). OHSS, characterized by ovarian enlargement, ascites, and hemoconcentration, represents a life-threatening complication. Hormonal imbalances may also contribute to pregnancy complications such as preeclampsia or gestational diabetes, underscoring the need for early clinical recognition and intervention.
Diagnosis of ADRs during hormonal support relies on vigilant clinical and laboratory monitoring. Serial assessment of ovarian size, serum estradiol, and hematocrit levels are pivotal in identifying OHSS. Diagnostic algorithms integrate clinical features with laboratory markers to enable timely detection and grading of adverse events. Pharmacovigilance systems, including spontaneous reporting and active surveillance, enhance early identification of rare or unexpected reactions.
Management of ADRs involves both preventive and therapeutic strategies. Dose adjustment, step-down protocols, and use of alternative agents (e.g., GnRH agonist trigger) mitigate OHSS risk. Supportive care with fluid management, anticoagulation for thromboembolism, and close monitoring of metabolic parameters are standard. Multidisciplinary collaboration ensures prompt escalation of care for severe complications, and patient education is central to early symptom reporting.
Recent advances include the adoption of individualized controlled ovarian stimulation, use of kisspeptin as an ovulation trigger, and development of long-acting progesterone formulations with improved safety profiles. Pharmacogenomic approaches are emerging to tailor hormonal support based on genetic risk factors. Digital health tools and real-time pharmacovigilance platforms are enhancing ADR detection and reporting, thereby refining patient safety measures.
Leading societies such as ESHRE, ASRM, and WHO recommend comprehensive drug safety surveillance encompassing pre-treatment risk assessment, protocol optimization, and structured follow-up. Guidelines emphasize patient selection, dose minimization, and evidence-based choice of hormonal agents. Active pharmacovigilance, including patient registries and mandatory reporting of severe ADRs, is advocated to inform clinical practice and policy.
Drug safety surveillance during embryology-related hormonal support therapies is a cornerstone of modern reproductive medicine. A nuanced understanding of pathophysiology, risk stratification, and current best practices enables clinicians to maximize therapeutic benefits while minimizing harm. Ongoing research, guideline development, and technological innovation will continue to advance safety monitoring, ensuring optimal outcomes for patients undergoing ART.
